Here you will find the Pre-Game / Pre-Practice Checklist Form (which should be completed for each game and/or practice) and the Incident Documentation Form (which should be completed after each incident). At this time, you will need to print each form for your use. Once completed these forms will need to be given to the Safety Officer and/or League President.
LL Insurance Form
Little League provides a secondary insurance for injuries that occur during a Little League Event. Your claim must first be submitted to your own insurance and then to the Little League Insurance. The Little League Insurance includes a deductible of $50.00 for the 2013 season. The first section of this form must be completed by the parent/guardian. The second section must be completed by one of our League Officials (President or Safety Officer).
When an injury occurs at one of our Little League Events, an Incident Report must be completed. If medical treatment is required, our Safety Officer and/or League President must be notified of the injury within 24 hours of the occurrence.
